Quantitative Myocardial Perfusion Assessment

CMR T1/T2 mapping reflects mechanistic severity in HFpEF beyond filling pressure

In 52 invasively confirmed HFpEF patients (multicentre DECIPHER study), corrected native T1 was associated with passive stiffness (β) and T2 with active relaxation (τ), independent of end-diastolic pressure. Mapping separated mild and severe diastolic dysfunction phenotypes that filling pressure did not; the authors consider this hypothesis-generating.

Contrast-free native T1 for diagnosis and risk in transthyretin cardiac amyloidosis

In 1,675 subjects from a national referral centre, native T1 ≤1,050 ms made cardiac involvement unlikely (93% sensitivity, 84% specificity) and ≥1,100 ms identified overt ATTR-CM (75% and 88%). ECV-calibrated T1 strata reproduced a 3-year mortality gradient; ECV remains preferred when contrast is available, and thresholds need local recalibration.
