Apical HCM: high-resolution apical cines

In apical HCM, wall thickness may not reach the classic diagnostic threshold. Dedicated thin, gapless, post-contrast cines of the apex help reveal it.

Tip by Julián Vega Adauy · jvega@incic.org

Apical short-axis cine: standard on the left, high resolution on the right.

Why

In apical hypertrophic cardiomyopathy, wall thickness may not exceed the classic diagnostic threshold. With standard cines the apex is poorly sampled and the disease can be missed.

How

  1. Add dedicated high-resolution cines of the apex.
  2. Use 6 mm slices with no gap, so no part of the apex is left uncovered.
  3. Tighten the FOV to the heart so the resulting voxel is small.
  4. Always acquire these cines after gadolinium to increase the signal-to-noise ratio.
