Home-made grey blood LGE: a partly nulled blood pool

With the usual TI the blood stays bright and can hide subendocardial enhancement. Pick a shorter TI on the TI scout: the blood turns grey and the subendocardial infarct stands out from the pool.

Top, the TI scout. Bottom, the same slice with TI 275 ms (bright blood, blue box) and TI 175 ms (grey blood, yellow box). The arrow marks inferolateral subendocardial enhancement.

Why

With the TI that nulls the myocardium, the blood in the ventricle stays bright. Subendocardial enhancement next to the pool blends in with the blood and can be missed.

How

  1. Acquire the TI scout.
  2. Choose the TI that leaves the blood grey, not the one that nulls the myocardium.
  3. Repeat late gadolinium enhancement with that TI in the doubtful slices.
